Requirements for a free or paid registration are the same:

The NCIR requires double-photo verification for all free or paid registrations.

The double-photo verification system guarantees the registration is being done by the proper person.

NCIR does a visual check of all submitted COVID vaccine forms and will reject any forms that appear to be altered in any way.

You will need the following 3 items to be able to register with NCIR:

1) Photo ID - driver’s license, governemnt issued ID, or passport. If you do not have one of these documents please submit the most official photo ID you have for review.

NOTE: the NCIR is not a government agency and has no interest in immigration status. If you are living in the USA, please register. If photo ID is an issue please tell us so in the NOTES section of the registration.

2) Photo holding photo ID: This is the double-photo verification step. Please take a selfie (or have someone take a photo) of you holding your Photo-ID next to your face so we can see both clearly at the same time. See example. This verifies you are submitting your COVID vaccination card on your own behalf and no one is filing a fake registration.

3) Your CDC COVID-19 Vaccination Record Card (or a vaccination record from a medical office)

Once you complete the registration process and upload your documents, you will be given the option of a free registration (which gives you a QR Code to store on your phone) or to continue with a Paid Registration and receive your official NCIR Vaccination Wallet Card.
